Telehealth Consent
Last updated: October 15, 2026
Before your teleconsultation with a The Med Club doctor, please read and accept two consents: (1) your informed consent to receive care through telehealth, and (2) your consent to the processing of your health information under the Data Privacy Act of 2012 (RA 10173).
Part 1. Informed consent for telehealth
What telehealth is
Telehealth means getting care from a licensed doctor who isn't in the same place as you, using video call, phone call or secure messaging. It may include sending your medical history, photos, test results or readings from devices like blood pressure monitors, and receiving advice, test requests or prescriptions electronically. Teleconsultations follow the Department of Health's telemedicine guidelines.
Possible benefits
- See a doctor from home or work, without travel or waiting time.
- Faster follow-ups, such as reviewing your lab results or adjusting a treatment plan.
- Easier access to care at times that suit you.
Possible risks and limits
- Your doctor can't do a full physical examination, which may limit the diagnosis. Your doctor may ask you to come in for an in-person visit or tests.
- Poor video, audio or internet connection may affect the consultation or cause delays.
- If your doctor doesn't have your complete records, there is a small risk of errors such as drug interactions or allergic reactions. Please share all your medicines and allergies.
- Although we use secure systems, electronic communication can never be 100% secure, and a breach of privacy is possible.
- Each treatment or medicine has its own risks and side effects, which your doctor will explain.
Your choices
- Telehealth is optional. You can choose an in-person consultation at our clinic, or a home service visit, at any time.
- Your doctor may decide telehealth isn't right for your concern and refer you to an in-person visit or another facility.
- You may stop a teleconsultation at any time.
Your responsibilities
- Give complete and truthful information about your identity, symptoms, medical history, medicines and allergies.
- Join from a private place, and confirm your identity with a valid ID if asked.
- Follow your doctor's instructions, and contact us or seek care if your symptoms get worse.
- Don't record the consultation without the doctor's consent (Anti-Wiretapping Law, RA 4200). Your doctor won't record it without yours.
Prescriptions and tests
- Your doctor decides whether a prescription is appropriate. A teleconsultation doesn't guarantee one.
- Dangerous drugs that need a special (S2 / yellow) prescription aren't prescribed through telehealth.
- If you need laboratory or imaging tests, they are done at our clinic or through our home service, and some specialized tests are sent to accredited partner laboratories.
Fees
Teleconsultation fees are shown when you book. Fees follow our Refund & Cancellation Policy: they are earned once the consultation starts, whether or not a prescription is given.
Not for emergencies
Telehealth isn't for emergencies. If you have chest pain, difficulty breathing, severe bleeding, loss of consciousness or any life-threatening symptom, call 911 or go to the nearest emergency room right away.
Part 2. Consent to process your health information
- Purpose: to provide your teleconsultation, create and keep your medical record, issue test requests and prescriptions, arrange follow-ups, and process payment, HMO or PhilHealth claims.
- Information: your personal details, such as your name, birthdate, sex and contact details. Also your sensitive personal information, such as your medical history, symptoms, photos you share, consultation notes, results and prescriptions.
- Sharing: only with your The Med Club doctors and care team, our laboratory and accredited partner laboratories, the dispensing pharmacy for your prescription, and your HMO, PhilHealth or company when you use them. Also with our secure technology providers, and with government authorities when required by law. We never sell your information.
- Retention: your medical records are kept for the period required by DOH regulations, at least 10 years from your last consultation, then securely destroyed.
- Your rights: you have the right to be informed, object, access, correct, request erasure or blocking, claim damages, data portability, and file a complaint with the National Privacy Commission. See our Privacy Policy.
